Life Teen - Youth Ministry
Permission Forms This year for the first time teens will be able to complete a registration form that is valid until August 15th following their graduation. Meaning that one youth ministry form will take you through your entire high school career. However, please note that permission forms for off site activities will still be mandatory for each event. You can download the current Life Teen - Youth Ministry registration right here. Complete all information and return to the youth ministry office to be entered into a drawing for 1 of 3 - $10 iTunes gift cards (teen must be present at the January 22nd Life Night to win). | Text Updates!
